The importance of a Care Circle

When a loved one needs help, we want to make sure they get the best care possible - meaning putting the client or patient at the centre of the care service is fundamental.

This is why a good Care Circle needs more than just a person’s carer, encompassing everyone in touch with them including family and friends, GPs and social care workers, agency managers and administrators, and if required, emergency services.

Each member of a Care Circle cares for a person in different ways, and all of them need access to the person’s health information. For example:

  • A GP may want to have an update on the client’s blood pressure to review their medication
  • A family member may want to know what they had for lunch
  • An agency manager may want to check the carer’s visit notes and care given
  • A paramedic who is called on site may need to access their medical history before making a decision on whether hospitalisation is needed

A patient could suffer from any number of ailments that interfere with their quality of life.
